Who can apply
['AHCCCS evaluates the applicable coverage category and financial and non-financial conditions. Family, pregnancy, adult, disability and long-term-care pathways differ.', 'Long-term care through ALTCS requires both financial and medical eligibility. Do not assume a general medical application requests all ALTCS services.']What help is available
Approved applicants receive health coverage for their category. Available plans depend on county and program, and provider networks differ. Check whether your doctor and pharmacy participate before selecting a plan.
Documents to prepare
Gather household, income and existing-insurance information. Use the appropriate application’s verification instructions. Applicants seeking disability, Medicare cost help or long-term care should ask about the additional records needed for that route.
How to get started
- Use AHCCCS’s How to Apply page.
- For many applicants, apply through Health-e-Arizona Plus or the linked DES Application for Benefits.
- The page identifies DE-103 for specified older, blind/disabled and Medicare-cost-sharing applicants. ALTCS has a separate registration and interview process.
- Save confirmation, respond to requests and use a community assistor if you need help completing the application.
After applying
Read the decision for category, effective date and enrollment details. When choosing a plan, compare its doctors and pharmacies in your county. Keep contact information current and follow renewal requests and reporting duties.
