Documents to prepare
Prepare household identities, income and existing insurance information. The application lists the proof needed for your coverage category. Ask for the long-term-care application if that is the care you need.
How to get started
- Apply using MyMedicaid, call 888-342-6207, visit a Medicaid office or application center, or use the paper application linked from LDH’s Get Covered page.
- Submit all requested verification and keep your confirmation.
- After approval, follow the notice about Healthy Louisiana plan enrollment.
After applying
Keep your address current and respond to renewal notices. Carry both the Medicaid and plan cards when applicable. The current Healthy Louisiana notice describes work requirements beginning January 2027; read future instructions and do not treat them as a present application requirement.
