Who can apply
HHS determines the appropriate Medicaid category. The Health and Wellness Plan has income and residency rules and is for adults not otherwise eligible for Medicaid or Medicare.
What help is available
Coverage includes medical care, prescriptions, mental health services and dental care. Health and Wellness Plan members may owe contributions after the first year unless applicable healthy-behavior steps are completed.
Documents to prepare
Have family income, existing health-insurance details, and applicants’ Social Security numbers or immigration document numbers available.
How to get started
Apply through the Iowa HHS Services Portal, a local HHS office or a participating health center. Telephone applications: 855-889-7985.
Submit any requested evidence and read the eligibility decision before making plan choices.
After applying
Approved applicants receive Medicaid and enrollment information. Check the assigned medical/dental plan, choice deadline and coverage date. As of October 2026, HHS announces community-engagement requirements for certain adult-plan members beginning by December 1, 2026; read notices for your circumstances.
