Nebraska Medicaid (Heritage Health)

Nebraska Medicaid applications can be submitted through iServe, by phone or on paper. Heritage Health is the managed-care arrangement for many approved members, not a separate shortcut to eligibility.

Who can apply

DHHS assesses coverage categories and financial circumstances. Children, pregnancy, age or disability, and Heritage Health Adult expansion have different rules. Current expansion work requirements and exemptions must also be considered for that category.

What help is available

Medicaid pays for qualifying healthcare services. The decision and health-plan packet identify coverage and enrollment arrangements.

Documents to prepare

  • Prepare applicant identity, household and income information.
  • Have current insurance and requested status records available.
  • Keep any relevant activity, medical or exemption information if applying under expansion.

How to get started

  1. Choose the Medicaid route

Apply through iServe or use phone/paper options on the Medicaid application overview.

  2. Answer eligibility requests

Respond for each applicant. The state publishes changed noncitizen eligibility effective October 1, 2026; ask DHHS to assess the current rule.

  3. Read coverage and plan notices

Review the individual decision and Heritage Health information, then confirm provider participation.

After applying

Keep iServe contact information current. The state’s expansion work-requirement page includes exemptions, temporary hardships and timing rules; follow your own verification notice.
