New Hampshire Medicaid & Granite Advantage

New Hampshire Medicaid and Granite Advantage applications begin through NH EASY or a DHHS district office. Each applicant’s coverage category and current requirements need agency review.

Who can apply

DHHS evaluates income, category-specific resource rules and nonfinancial conditions such as New Hampshire residence. Categories include children under 19, pregnancy, caretaker relatives, former foster youth, older adults and people with disabilities. Granite Advantage covers eligible adults aged 19 through 64. Disability and long-term-care cases may need additional assessments.

What help is available

Approved medical coverage pays for qualifying services. The decision and plan information establish the coverage start and how to access care.

Documents to prepare

  • Identity, residence and household details requested by DHHS.
  • Current income and existing health coverage.
  • Resource records if required for your eligibility category.
  • Medical or disability evidence if the category requires a determination.

How to get started

  1. Start the application

Use NH EASY or request a paper application from a DHHS district office.

  2. Complete verification

Identify everyone needing coverage and respond to individual document requests.

  3. Review the outcome

Read each person’s decision and any plan-selection or renewal instructions.

After applying

Read each person’s coverage start date, plan and renewal instructions. Keep contact details current and respond to verification requests. If you disagree with the decision, follow the agency’s administrative-appeal instructions and the deadline on your notice.

Common questions

Can family members have different decisions?

Yes. Coverage categories and individual circumstances differ.

Should I use an old Granite Advantage income chart?

No. Ask DHHS for the current year and household-specific assessment.

Who can answer an application question?

Call the DHHS Customer Service Center at 844-275-3447, Monday through Friday, 8 a.m.–4 p.m. Eastern, or contact a district office.

What if income is too high but medical bills are substantial?

Ask about In and Out Medical Assistance. Some applicants can qualify after eligible medical expenses reach an assigned spenddown; it is not automatic coverage for everyone above the income limit.
